Terms and Conditions

If you need to cancel your assessment, you will need to notify me by email at jane@learningtoempower.co.uk

Cancellation charges will be applied as follows:

• Cancellation within 7 days of a booked session – 30% of the full fee
• Cancellation within 14 days of a booked session – Loss of deposit. 

In the unlikely event that I will need to cancel due to illness, I will rearrange a mutually convenient date with you as soon as possible or arrange for a full refund.

 Written permission from all individuals with parental responsibility is mandatory before any assessment can proceed. I reserve the right not to proceed with an assessment once all questionnaires have been received and reviewed if, in my professional judgement, it does not appear to be the most appropriate course of action. In this case, a full refund, including deposit, will be given. 

All questionnaires must be completed honestly and accurately in order to provide the clearest and most comprehensive picture of your child. It is not possible to proceed with an assessment if previous assessments, reports or other information that is relevant to the assessment has been withheld.

Completion of the school questionnaire is mandatory before any assessment can take place.

Following the assessment, please allow up to 21 days for the report to be sent to you.

A valid eye test conducted within the past 12 months is a pre-requisite for the assessment. Completion of the parents' questionnaire, school questionnaire and visual difficulties screening questionnaire are mandatory prior to the assessment.
